Instructions:
1. Stock held by your broker in street name:
- To obtain transfer information and instructions, inform the Museum of a pending stock transfer and to ensure proper gift crediting, please contact, or instruct your stock broker to contact, The Jewish Museum\'s Development Department at 212.423.3304, fax 212.423.3233, or development@thejm.org. Please supply the donor\'s name, the street name of the stock, number of shares being transferred and how you would like the gift to be used.
- Ask your broker to wire transfer the gift directly to The Jewish Museum\'s account. They will need to call The Jewish Museum at 212.423.3304 to obtain the DCT number and account number. The Jewish Museum\'s brokerage firm is:

2. Stock Certificates in your name:
- Please send (or have your broker send) the stock certificates via certified
mail to The Jewish Museum, Attn: Development Department, with a letter indicating how you would like the
gift to be used. DO NOT sign the reverse side of the certificate.
- In an envelope separate from the certificate, please send a completed Release
Form and Stock Power Form. Note: Both forms must be signed and witnessed and
the Release Form must be notarized.
- Your gift of stock to The Jewish Museum is not complete, for tax purposes,
until the Museum receives all of the required documents, in properly
executed form.
3. Stock Certificates in The Jewish Museum\'s name, held by you:
- Send the certificates via certified mail to The Jewish Museum, Attn: Development Department, with a letter
of instruction indicating how you would like the gift to be used.
